This easy mail/contact form allows your users to send you mail directly from your website and can be easily edited with CSS.
by just editing the red marked image name e.g something like “load_image.png” –> if(http.readyState == 1) {Download the .zip archive and extract it to the place where you want it to be on your PC.
Open up the file config.php.
Search for these lines:
/* CONFIG
----------*/
// WEBMASTERS EMAIL
$target = "Insert here your E-mail where you'd like to get the messages.";
// WEBMASTERS NAME
$targetname = "Insert here your official name or just an alias.";
// ENABLE SUBJECT PREFIX (true or false)
$subjprefixe = true; // SET TO FALSE TO DISABLE PREFIX
// SUBJECT PREFIX
$subjprefix = "Insert here your Subject Title as you want it.";
The line that is red marked you’ve to insert your own e-mail between the “”, something like this: you@yourdomain.com or myemail@gmail.com.
The line that is green marked you’ve to insert your own name between the “” it also can be an alias.
The line that is blue marked you’ve to insert the subject like how you want it, e.g “Message from [yoursite]”
Now when you’ve done that go to contact.php and replace with your header include ( and replace also with your header include. 
Now you can upload the content to your server, and vóíla
